Severe bad breath removal, senile periodontitis, dry mouth, and even front teeth spacing —

All about aging oral care explained by Dr. Kim Yoonsang of Maeum Perio Dental Clinic

As aging begins, various organs in our body gradually lose function, but changes in oral health are felt particularly quickly. Receding gums, tooth sensitivity, spacing of the front teeth, and worsening bad breath are common concerns among middle‑aged and older adults.

This article is based on explanations from Dr. Kim Yoonsang, former director of Wonkwang University Dental Hospital and current head of Maeum Perio Dental Clinic, providing an in‑depth overview of oral problems caused by aging and their solutions. It covers methods to reduce severe bad breath, management of senile periodontitis, treatment of dry mouth, and approaches for correcting front‑tooth spacing.

Why do gum disease and bad breath worsen with aging?

Dr. Kim Yoonsang explains that “aging is ultimately the result of lifelong accumulated bacterial exposure.”

We eat many times a day, swallow saliva, speak, and breathe, constantly coming into contact with bacteria in the mouth. No matter how well we take care of our oral health, chronic gum inflammation gradually accumulates, eventually causing the gums to recede and the spaces between the teeth to widen.

✔ As the gums recede, spaces form between the teeth

When the gums recede, the roots of the teeth become exposed, and the spaces between the teeth widen, causing food to get stuck more easily.

These food residues become a source of nourishment for bacteria, and as the bacteria multiply, sulfur compounds that cause bad breath increase.

✔ Tooth mobility → Food impaction → Inflammation → Odor

When the bone that supports the teeth weakens due to aging, the teeth begin to move slightly.

This slight movement makes food get stuck more easily, which then causes inflammation and leads to odor.

Especially when periodontitis has progressed, bad breath can become even stronger.

✔ Bad breath originates not only in the mouth but also from the digestive and respiratory systems

Bad breath is not just an oral problem.

  • Odor rising from the stomach
  • Tonsil and nasal issues
  • Respiratory diseases

Although there are various causes, Dr. Kim Yoonsang emphasizes that reduced saliva production is the biggest issue in aging.

Why dry mouth is a key cause of severe bad breath

As we age, the function of the salivary glands declines, reducing saliva production.

Saliva doesn’t just moisten the mouth; it acts as an important protective barrier that suppresses bacteria and keeps the oral cavity clean.

✔ Problems that occur when saliva decreases

  • Dry mouth makes speaking and eating uncomfortable.
  • Reduced ability to suppress bacteria
  • Inflammation occurs easily
  • Increase in odor‑causing gases as bacteria become more active

In short, dry mouth is a key cause of severe bad breath.

Methods to reduce dry mouth

Director Kim Yoonsang says, “It’s difficult to completely prevent dryness, but minimizing it is important.”

✔ Using products that stimulate saliva production

  • Salivary‑gland‑stimulating candy
  • Sugar‑free gum
  • Saliva stimulant

These products stimulate the salivary glands and help increase saliva production.

✔ Use of artificial saliva

Using artificial saliva when the mouth becomes too dry can reduce bacteria activation caused by dryness.

✔ Drinking water frequently

Older adults tend to drink water less frequently on their own, so they need to manage it consciously.

✔ Regular dental visits

  • Checking for tooth damage
  • Cavity treatment
  • Gum inflammation management
  • Scaling and periodontal treatment

These basic forms of care play a major role in reducing bad breath.

Why do front teeth start to gap and become sensitive?

As aging progresses, changes in the front teeth become especially noticeable.

Director Kim Yoonsang explains that “the front teeth are the area where accumulated wear and damage from lifelong use appear most clearly.”

✔ Reasons why the front teeth start to gap

  • Gum recession weakens the support of the teeth
  • The teeth slightly loosen, causing the alignment to shift
  • The height of the tooth decreases due to wear
  • Cracks or chipping lead to structural weakening

As these changes accumulate, gaps form between the front teeth and sensitivity develops.

How can gapped front teeth and sensitivity be treated?

✔ 1) Restoration of tooth damage

In cases of wear, cracks, or chipping

  • Resin restoration
  • Prosthetic treatment

Through these treatments, tooth sensitivity can be reduced and cavities can be prevented.

✔ 2) Gum treatment

Reducing gum inflammation and rebuilding the gums can improve tooth support and prevent the worsening of front‑tooth spacing.

✔ 3) Periodontal orthodontics

When the alignment of the front teeth is significantly distorted,

Periodontal orthodontics, which carefully considers the condition of the gums and bone, is needed rather than general orthodontics.

Because the bone condition is weaker in older adults, treatment should proceed slowly and safely.

Key summary for reducing severe bad breath

  • As aging causes the gums to recede and spaces to form between the teeth, bad breath becomes more severe.
  • Reduced saliva production is the biggest cause of worsening bad breath.
  • Managing dry mouth is very important.
  • Front‑tooth spacing and sensitivity are caused by lifelong accumulated wear and changes in the gums.
  • Regular periodontal treatment and periodontal orthodontics can be helpful.

If bad breath worsens or gum and tooth problems persist,

It is necessary to consult a dental specialist to accurately assess your condition.
